About two out of every three of the estimated 7 million Americans living with Alzheimer's are women, and that context argues for taking cognitive symptoms seriously rather than filing them under stress by default. That's the case for repeat or timed testing over one-and-done labs, particularly when the picture is muddied by an overlapping condition.

What to bring to a clinical visit — and how to make the case for thorough evaluation

Women who feel dismissed, told their symptoms are just stress, just aging, just anxiety, without ever getting a hormonal workup, are not imagining a pattern. It's a well-documented one in perimenopause care, and the fix is preparation.

Before a visit, a few things are worth having written down rather than half-remembered under fluorescent lighting: menstrual cycle changes across at least two or three cycles (dates, flow, duration), a symptom log noting frequency and time of day, sleep disruption patterns including whether night sweats are present, and a plain account of what the symptoms actually prevent someone from doing.

That last point matters more than it sounds. Saying "this is stopping me from completing normal activities" lines up directly with the FDA's functional definition of severity, and it gives a clinician something more actionable than "it's really bad."

On testing: under 45, FSH and estradiol testing is clinically appropriate to help clarify whether POI may be present. At any age, testing makes sense if symptoms aren't following an expected pattern, if a treatment already tried isn't working, or if a clinician is attributing everything to another cause without checking whether hormones play a role. If testing does get ordered, timing changes what it can tell you, and tracking levels over time rather than leaning on one draw surfaces patterns that a single test simply can't show.

Women who show up with documented patterns, not vague impressions, are simply better positioned to get both.
